STEP 1: checklist of required documents
- Original passport issued within the last year
- Two passport size photos 2"X 2" (must be taken within last 2-3 months), no glasses
- One Application DS-5504 (click here to fill out online or download below), we have to submit it to the passport agency within 5 business days.
- One Letter of Authorization- (download below) once you sign it, we have to submit it to the passport agency within 5 business days
- Round trip itinerary showing international travel within 14 days. If traveling later than in 14 days, download & fill out proof of departure sample letter below.
- AVS Order Form (download below)
- For name change: a certified copy of the marriage certificate, divorce decree or a court order (this document will be returned to you upon passport completion)
- For data correction: a certified copy or original document showing your data printed correctly (for example, if your date of birth or place of birth were printed incorrectly, original birth certificate will be required)
