step 1: gather the following documents according to the trip

TOURIST REQUIREMENTS

Original signed passport- must have a minimum of 1yr validity left and have 2-3 clear visa pages
One passport photo 2"x 2" (must be taken within last 2-3 months)
One visa application form (download below). Application must be typed in capital letters and all fields must be filled in (put "N/A" or "NONE" if any questions do not apply to you). Any hand-written or double-sided application will be re-typed for a $25 fee
One AVS Order Form (download below)
Copy of applicant's driver's license or state-issued ID
Copy of flight itinerary
Copy of hotel reservation
Copy of green card for non-US citizens

BUSINESS REQUIREMENTS
