TOURIST REQUIREMENTS

Original signed passport- must have a minimum of 6mo of validity left and have 2-3 clear visa pages
One passport photo 2"x 2" (must be taken within last 2-3 months)
One visa application form (download below). Applicant's signature must be notarized on page 2
One AVS Order Form (download below)
Copy of flight itinerary
Copy of hotel reservation or invitation from an individual in Afghanistan along with a copy of their ID
Copy of green card for non-US citizens

BUSINESS REQUIREMENTS
